311 Rib Fractures in Geriatric Trauma: A Review of 1,037 Cases at a Single Level I Trauma Center

OBJECTIVES/GOALS: Rib fractures are common traumatic thoracic injuries and are associated with high rates of morbidity and mortality. In those age ≥ 65, the rate of these complications double. This study sought to identify the extent to which injury-related predictors influence clinical outcomes in geriatric patients with rib fractures. METHODS/STUDY POPULATION: A retrospective 5-year review was performed of a single Level 1 Trauma center registry. Geriatric patients (≥65 years of age) diagnosed with rib fractures from January 1, 2014 to December 31, 2019 were included. The primary outcome of interest was in-hospital mortality. Secondary outcomes included hospital and intensive care unit length of stay (HLOS and ICU LOS, respectively) and discharge disposition, as a surrogate for loss of independence. Further, subgroup analysis based on number of rib fractures (i.e. <4 and ≥4 rib fractures) was performed. RESULTS/ANTICIPATED RESULTS: 2,134 adult trauma patients were admitted with at least one rib fracture. Of these, 1,037 (49%) were ≥ 65 years old. This cohort had a mean age of 78.6 years old, injury severity score (ISS) of 11.4, HLOS of 7.4 days and 29% required ICU care with mean ICU LOS of 1.9 days. Only 36% were discharged home compared to 64% who were discharged to a care facility and thus had a loss of independence. Overall mortality was 6.3%. Compared to survivors, non-survivors had a higher ISS (19.3 vs. 10.8, p = < 0.0001) and longer ICU LOS (7.1 vs. 6.5 days, p = 0.04). Analysis based on number of rib fractures showed that those with ≥4 rib fractures had significantly higher mortality (8% vs. 4%, p = 0.008), longer HLOS (8.7 vs. 6.1 days, p < 0.0001), longer ICU LOS (2.6 vs. 1.3 days, p < 0.0001), and significantly lower discharge to home (32% vs. 39%, p = 0.02). DISCUSSION/SIGNIFICANCE: To our knowledge, this is the largest single-center study of geriatric patients with rib fractures. In this study, the observed mortality in patients ≥65 years of age was 6.3% which represents a lower mortality rate than historically reported. Despite this, only 36% were able to be discharged directly to home.
“Do I Have a Memory Problem? I Can’t Recall”: An Evaluation of Measurement Invariance in Subjective Reporting of Memory Symptoms among Persons with and without Objective HIV-Associated Memory Impairment

Objective:
Memory symptoms and objective impairment are common in HIV disease and are associated with disability. A paradoxical issue is that objective episodic memory failures can interfere with accurate recall of memory symptoms. The present study assessed whether responses on a self-report scale of memory symptoms demonstrate measurement invariance in persons with and without objective HIV-associated memory impairment.
Method:In total, 505 persons with HIV completed the Prospective and Retrospective Memory Questionnaire (PRMQ). Objective memory impairment (n = 141) was determined using a 1-SD cutoff on clinical tests of episodic memory. PRMQ measurement invariance was assessed by confirmatory factor analyses examining a one-factor model with increasing cross-group equality constraints imposed on factor loadings and item thresholds (i.e., configural, weak, and strong invariance).
Results:Configural model fit indicated that identical items measured a one-factor model for both groups. Comparison to the weak model indicated that factor loadings were equivalent across groups. However, there was evidence of partial strong invariance, with two PRMQ item thresholds differing across memory impairment groups. Post hoc analyses using a 1.5-SD memory impairment cutoff (n = 77) revealed both partial weak and partial strong invariance, such that PRMQ item loadings differed across memory groups for three items.
Conclusions:The PRMQ demonstrated a robust factor structure among persons with and without objective HIV-associated memory impairment. However, on select PRMQ items, individuals with memory impairment reported observed scores that were relatively higher than their latent score, while items were more strongly associated with the memory factor in a group with greater memory impairment.
Predation of nesting Thick-billed Parrots Rhychopsitta pachyrhyncha by bobcats in northwestern Mexico

We report on what appear to be increasing predation events on nesting Thick-billed Parrots Rhychopsitta pachyrhyncha. Thick-billed Parrots are classified as ‘Endangered’ and their seasonal breeding range is restricted to increasingly fragmented and degraded high elevation mixed conifer forest habitat within the Sierra Madre Occidental region of north-western Mexico. Predation of established breeding pairs has recently contributed to the ongoing decline of Thick-billed Parrot populations by removing mature birds with high reproductive value, which has associated consequences for future recruitment. We observed increasing predation events on nesting Thick-billed Parrots by bobcats Lynx rufus accompanied by kittens throughout the 2018–2019 breeding seasons, and we speculate that recent reductions in bobcat habitat have pushed them into new ranges where they are supplementing their diet with nontraditional prey items.

OBJECTIVES/SPECIFIC AIMS: To assess the association between probable OSA and the sudden unexpected death in epilepsy (SUDEP-7) risk profiling index in monitored adult inpatients with epilepsy. METHODS/STUDY POPULATION: We analyzed 49 consecutive adults (>18 years) with refractory epilepsy admitted to our inpatient epilepsy monitoring unit. The SUDEP-7 inventory was performed for all subjects. Probable OSA was identified using overnight oximetry, the Sleep Apnea Sleep Disorder Questionnaire (SA-SDQ), and STOP-BANG inventory. RESULTS/ANTICIPATED RESULTS: Thirty-nine percent of participants screened positive for probable sleep apnea. Patients with high SUDEP-7 scores were more likely to have a positive screen for OSA. DISCUSSION/SIGNIFICANCE OF IMPACT: OSA is an independent risk factor for sudden cardiac death. OSA may be a hitherto unrecognized contributor to sudden death risk in epilepsy. Further studies determining the relationship between OSA, neural circulatory control and SUDEP are warranted.
Effect of Retrieval Practice on Short-Term and Long-Term Retention in HIV+ Individuals

Objectives: Episodic memory deficits are both common and impactful among persons infected with HIV; however, we know little about how to improve such deficits in the laboratory or in real life. Retrieval practice, by which retrieval of newly learned material improves subsequent recall more than simple restudy, is a robust memory boosting strategy that is effective in both healthy and clinical populations. In this study, we investigated the benefits of retrieval practice in 52 people living with HIV and 21 seronegatives. Methods: In a within-subjects design, all participants studied 48 verbal paired associates in 3 learning conditions: Massed-Restudy, Spaced-Restudy, and Spaced-Testing. Retention of verbal paired associates was assessed after short- (30 min) and long- (30 days) delay intervals. Results: After a short delay, both HIV+ persons and seronegatives benefited from retrieval practice more so than massed and spaced restudy. The same pattern of results was observed specifically for HIV+ persons with clinical levels of memory impairment. The long-term retention interval data evidenced a floor effect that precluded further analysis. Conclusions: This study provides evidence that retrieval practice improves verbal episodic memory more than some other mnemonic strategies among HIV+ persons. (JINS, 2017, 23, 214–222)

Eleven patients with tardive dyskinesia were treated with lithium carbonate in a placebo-controlled double-blind crossover study. No significant effect of lithium on either tardive dyskinesia or blood prolactin concentrations was demonstrated, but 5 patients developed pseudo-Parkinsonian features.

While much work is proceeding with regard to the preservation and restoration of audio documents in general and compositions for tape in particular, relatively little research has been published with regard to the issues of preserving compositions for live electronics. Such works often involve a distinct performance element difficult to capture in a single recording, and it is typically only in performance that such works can be experienced as the composer intended. However, performances can become difficult or even impossible to present over time due to data and/or equipment issues. Sustainability here therefore refers to the effective recording of all the information necessary to set up the live electronics for a performance. Equally, it refers to the availability of appropriate devices, as rapid technological change soon makes systems obsolete and manufacturers discontinue production. The authors have had a range of experience re-working performances over a number of years, including compositions by Luigi Nono and Jonathan Harvey, amongst others. In this paper we look at the problem as a whole, focusing on Jonathan Harvey's works with electronic elements, which span some twenty-six years, as exemplars of the types of problems involved.

Irregular satellites have eccentric orbits that can be highly inclined or even retrograde relative to the equatorial planes of their planets. These objects cannot have formed by circumplanetary accretion as did the regular satellites which follow un-inclined, nearly circular, pro-grade orbits. Instead, they are likely products of early capture from heliocentric orbit. The study of the irregular satellites provides a unique window on processes operating in the young solar system. Recent discoveries around Jupiter (45 new satellites), Saturn (13), Uranus (9), and Neptune (5) have almost increased the number of known irregular satellites by a factor of ten and suggest that the gas and ice giant planets all have fairly similar irregular satellite systems. Dynamical groupings were most likely produced by collisional shattering of precursor objects after capture by their planets. Jupiter is considered as a case of special interest. Its proximity allows us to probe the fainter, smaller irregular satellites to obtain large population statistics in order to address the questions of planet formation and capture.
